Abstract
We report a case of ischemic colitis with a discharge of sigmoid colon necrosis after aorto-femoral Y graft. A 65-year-old male with arteriosclerosis obliterans complained of rest pain in the left lower leg. Aorto-femoral Y grating was carried out. On the second postoperative day, he had slight melena, and on the 21st day he passed colonic necrosis, 16 cm in length and cylinder shaped. Drainage was performed on the 22nd day and colostomy was established. He became better after the re-operation. In the other reports, discharge of intestinal necrosis was also observed several weeks after a bypass operation. As the pathogenesis of this rare manifestation, slow and gradual advance of intestinal ischemia, resulting necrosis was suspected.
