RESULTS IN THE TREATMENT OF INTRAHEPATIC GALLSTONE

Abstract

Up to April 1991, 53 patients with intrahepatic gallstone were treated surgically at the department. For these patients, the effect of the operation and prognosis after operation were studied. The following findings could be drawn; 1) The patients who had the intrahepatic gallstone in the bilateral lobes needed many operations. 2) The rate of residual stones were higher in the patient having the stones in the bilateral lobes than in those having the stones in the unilateral lobe. 3) Two patients died of cholangiocarcinoma that occurred during the period of the treatment or the follow-up observation of intrahepatic gallstone. 4) Even the patients needed many operations could have a satisfactory prognosis owning to the intensive endoscopic lithotomy after the operation.
Therefore, we consider that it is necessary for the treatment of the patient with intrahepatic gallstone to make an accurate diagnosis of the abnomality of the biliary tracts at the first operation in order to avoid many operations and to treat with the endoscopic lithomy for residual stones after the operation. It is difficult to predict the postoperative occurrence of the cholangiocarcinoma, but this retrospective study indicates that we should check it strictly, i.e., by means of the direct cholangiography.
