Abstract
The septum formation of the extra-hepatic bile duct is a very rare anormaly. Only 35 such cases have been reported in the Japanese literature up to now. In this paper, a case of membranous septum formation of the common hepatic bile duct is reported. A 51-year-old woman presenting with upper abdominal pain had liver dysfunction. ERCP shows a stenosis of the common hepatic duct and many stones in bilateral intra-hepatic bile ducts. The laparotomy was performed with a diagnosis of cancer of the upper bile duct. Any tumor was not found, and a cholangio-fiberscope revealed the membranous septum of the common hepatic duct with a small hole at its center. The septum was resected and many stones were taken out. After confirming that there was no remnant stone, the operation was completed. Historogically, the septum was comprized of fibroblast and collagen fiber and no malignant findings were noted. Postoperative course was unevectful, accompanying by no recurrent stones.
