Abstract
This paper describes 3 cases of distal aortic arch aneurysm which were successfully managed by graft replacement or graft patch aortoplasty by using hypothermic selective cerebral perfusion (HSCP) as an adjuvant procedure. Case 1 was admitted to the hospital because of an abnormal shadow on a chest X-ray film, and case 2 and 3, because of horseness. All 3 cases were diagnosed as distal aortic arch aneurysm by chest CT and angiography. Aneurysm was closed by graft patch aortoplasty in case 2, while graft replacement was performed in case 1 and 3. Case 1 and 2 were approached through median sternotomy. Case 3 was approached through median sternotomy continued with left anterior thoracotomy so called door open method. With these approaches using HSCP, we could get good view at operation and anastomosis was able to be done at the normal portion of the aorta. No cerebral complication occurred.
