A CASE OF RECTAL CANCER ASSOCIATED WITH RECTAL TUBERCULOSIS

Abstract
This is a case report of a patient with rectal cancer associated with rectal tuberculosis. A 66-year-old man was seen at the hospital because of abdominal distension. There was no history of tuberculosis and anal fistula, nor family history of tuberculosis. Digital examination of the rectum, barium enema, and colonic endoscopy revealed a whole-circumference stenosis 3 to 4 cm distant from anal verge. Biopsy findings of the lesion showed no malignancy, but the intestinal obstruction increased. Under a suspecion of rectal carcinoma, proctotomy was performed. In histopathological examination of the excised specimen, a rectal carcinoma associating with rectal tuberculosis was found. Histologic type of the carcinoma was mucinous adenocarcinoma. No carcinoma was found on the mucous membrane of rectum. It might arise in the anal gland.
Carcinoma in an association with tuberculosis of the rectum rarely occurs in Japan, and only 55 cases have been reported since 1930 when Harada first described it. Almost cases had a carcinoma locating from the ileocecum region to right hemicolon where is a common site for tuberculosis. The association of carcinoma and tuberculosis in the rectum like this case has not been reported, excepting 2 cases which were associated with cancer of anal fistula.
