Abstract
Ruptured hepatocellular carcinoma (r-HCC) is liable to cause hemmorhagic shock and its prognosis is very poor. Recent three patients with r-HCC who could be saved by lipiodol transarterial embolization (L-TAE), are described. Moreover two of the three patients were added second stage hepatectomy and had favorabel surgical outcomes.
All cases complained of a sudden onset of severe abdominal pain and significantly developing anemia. Case 1 was a 64-year-old man. Reight lobectomy of the liver was performed on the 40th day after TAE. He is alive as of 2 years and 8 months after the onset of the disease. Case 2 was a 65-year-old man. Superior segmentectomy was performed on the 90th day after TAE. He is alive as of 9.5 months. Case 3 was a 72-year-old man who could not undergo hepatectomy because of extremely poor liver and respiratory functions after TAE. He died 108 days after the onset.
In the treatment of r-HCC, TAE followed by hepatectomy for selected patients whose general conditions are recovered with sufficient hepatic reserve may be recommended for improve the short or long term prognosis.
