SURGICAL NOTES ON SIMULTANEOUS Y-GRAFT REPLACEMENT AND GASTROINTESTINAL OPERATION

Abstract
In 31 cases of AAA and 8 of ASO, a total of 39, Y-graft replacement was simultaneously performed with an operation for a gastrointestinal disease. These operations included cholecystectomy in 17 cases; cholecystectomy with appendectomy in 4; appendectomy in 15; diverticulectomy for 3 diverticula in the ileal region because of probable diverticulitis and perforation of them following operation in one; Hartmann's operation because of lasting bleeding from an upper rectal cancer and large aneurysm in one; and excision of the transverse colon because of localized ischemia in the transverse colon with was found at freeing of severe adhesion of it after operation for gastric cancer in one. Of 21 cholecystectomy cases 19 has stones in the gallbladder and the remaining 2 were of adenomyomatosis. Intraoperative findings of appendectomy cases included catarrhal appendicitis in 13 cases; coprolith in 3; and besides, supprative appendicitis in one; and suspected cancer of the appendix in 2. The latter 3 cases were of mucous cyst histopathologically.
Simultaneous operation of Y-graft replacement under an asceptic field with gastrointestinal operation has been performed for some cases which were judged proper candidates for this method, with a favorable postoperative course. It is emphasized, however, that this simultaneous operation should be performed carefully by a skilled surgeon.
