Abstract
A case of spontaneous rupture of the esophagus with a gastric cancer, which was successfully cured by conservative therapy, is experienced. A 65-year-old man was seen at the hospital because of sudden onset of hematemesis. By emergency CT and endoscopic examination, an advanced gastric cancer and hydrothorax were found. An emergency operation was performed. Esophagography after partial gastrectomy revealed a leakage in the lower portion of the esophagus. A diagnosis of spontaneous rupture of the esophagus with its resultant pyothorax was made. Because of his poor pyothorax condition after the operation, he was treated conservatively and cured.
As a general rule, spontaneous rupture of the esophagus requires early diagnosis and emergency surgical management. However for patients in poor general condition like this case, conservative therapy should be selected.
