The resistance-declined portion in the lumbar region is well known as the lumbar triangle anatomically, and it is extremely rate that hernia occurs in the same region. We experienced a case of superior lumbar hernia presented with a left frank mass. A 70-year-old man was seen at the clinic because of left frank mass which was noticed about one year before. CT revealed a tumor protruded from the defect of the aponeurosis in the lumbar dorsal region. It was inferred that hernia content was the fat around the kidney. With the diagnosis of superior lumbar hernia, herniorrhaphy was performed. Hernia defect, 3cm diamiter and bounded superiorly by the 12th rib, medially by the quadrates lumborum muscle, and laterally by the external oblique, was closed with interrupted nonabsorbable sutures without tension. The postoperative course was uneventful. There has been no recurrence, as of 5 months after the operation.
