J-pouch construction with low anterior resection for two advanced and two early rectal cancer localized below the peritoneal reflection was performed to avoid eternal colostomy and improve postoperative function of evacuation. The reconstruction procedures were colo-anal anastomosis in one case and colo-rectal anastomosis in other three cases. Furthermore, diverting transversecolostomy was carried out in two cases. All patients had good postoperative course without any complications.
In terms of surgical radicality and quality of life after operation, J-pouch construction with low anterior resection appears to be an effective procedure. The procedure promises to be used in wider range of indications in future.
