Abstract
We experienced a case of primary adenocarcinoma with a goblet cell carcinoid of the appendix.
A 62-year-old woman visited another hospital because of right lower abdominal pain, and was referred to the hospital. Tenderness and rebound tenderness were localized in the ileocecal region, where a tumor was palpable. Laparotomy was performed under general anesthesia with preoperative diagnosis of perityphilc abscess. We found the appendix red and swollen, and performed an ileocecal resection with lymph node dissection, suspecting it of malignancy. A histopathological examination revealed well differentiated adenocarcinoma in the entire appendix and a solid nest of goblet cell carcinoid at the distal portion of the appendix.
Goblet cell carcinoid of the appendix is rare, and only 55 cases have been reported in Japan. We cannot find a case of both goblet cell carcinoid and adenocarcinoma of the appendix as in our case.
The patient was followed up on an ambulant basis. Liver metastasis was found 14 months after the surgery, and was managed by a right lobectomy of the liver. It was metastasis of goblet cell carcinoid.
