Abstract
A case of idiopathic perforation of the transverse colon is reported. A 65-year-old woman with pyoderma gangrenosum was referred to our department because of abdominal pain. There were severe tenderness in the entire abdominal region and signs of peritoneal irritation.
Intraperitoneal free air was detected by computed tomography. Emergency operation was carried out under a diagnosis of diffuse peritonitis due to perforation of the colon. Laparotomy findings revealed a punched out perforation 3cm in size in the transverse colon. A partial resection of the transverse colon including perforated portion was performed. Histologic examination showed that mucosa and muscle layer was sharply cut off without indentation, and there were some neutrophil cell infiltrations, without granulation nor microabscesses. The histological diagnosis was identical to the category of idiopathic perforation of the colon. Although idiopathic perforation of the transverse colon is a rare disease, we have to keep the disease in mind as a probable differential diagnosis.
