Abstract
We report here a rare case of cystic hypersecretory carcinoma of the breast.
A 66-year-old woman visited our hospital because of a right breast mass. Physical examination revealed an oval, elastic hard mass measuring 38mm in diameter in the upper and outer quadrant of the right breast. Mammography demonstrated an irregular mass with focal asymmetric density. The tumor appeared to be a multicystic lesion ultrasonographically, and aspiration cytology revealed sheets of ductal epithelial cells without atypia in abundant eosinophilic fluids. Open biopsy was carried out. Histopathologic examination revealed multiple cysts of different sizes containing an eosinophilic material resembling thyroid colloid. The epithelial cells of these ducts showed partial micropapillary patterns with hyperchromatic nuclei, and a diagnosis of cystic hypersecretory carcinoma was made. Although the lesion did not appear to be invasive, surgical margins were positive for carcinoma. Therefore mastectomy was carried out. In mastectomy material, there was no evidence of invasive carcinoma except for a single micrometastasis in axillary lymph node. Histopathologically, the final diagnosis of the tumor was invasive cystic hypersecretory carcinoma.
