A three-year-old girl was admitted because of petechiae and fever. The white blood cell count on admission was 21,400/μl with 78% blasts. Bone marrow aspirate showed a nucleated cell count of 36×104/μl with 55.6% blasts, 2.0% promyelocytes, 12.0% myelocytes and 8.8% neutrophils. A diagnosis of acute myeloblastic leukemia (M2 in FAB classification) was made. Auer rods were observed not only in blasts but also in mature neutrophils. A complete remission was obtained by a combination consisting of cyclophosphamide, vincristine, cytosine arabinoside and prednisolone. Auer rods in mature neutrophils almost disappearead after complete remission as previously reported but the number of them increased during the induction chemochemotherapy therapy.
