This research aims to clarify factors related to care levels after 5 years in community-living elderly people certified as lower care levels, and to discover preventive approaches to nursing-care. The analysis covered1,598 persons certified as requiring care in 2004 (Support required: 939; Care level 1: 659). Based on a univariate analysis of basic attributes and physical, psychological, and social items, we conducted a multiple logistic regression analysis of items having a significant correlation with care levels after 5 years. For both the group “Support required” and the group “Care level 1”, the care levels after 5 years were associated with TMIG-Index of Competence and over 75 years old, toileting accidents. For the group “Support required”, subjective evaluation of health was unhealthy and no hobbies were additional factors associated with care levels after 5 years. For the group “Care level 1”, fall experience within the past one year was additionally associated with care levels after 5 years.
Elderly people with lower care levels need prevention of toileting accidents such as incontinence. The group “Support required” needs activities with hobbies. The group “Care level 1”needs prevention of fall.
