Report on 60 Cases of Acute Small Craniotomy STA-MCA Bypass Using Duraplasty Performed with DuraGen®

Abstract

Postoperative complications of superficial temporal artery-middle cerebral artery (STA-MCA) bypass surgery are known to include cererospinal fluid (CSF) leakage, wound healing difficulties, postoperative hemorrhage, etc. Our hospital has been using an artificial dura mater made of collagen (DuraGen®) since October 2019 for dural formation in STA-MCA bypass surgery performed in the acute phase of cerebral infarction, to shorten operation time. The same technique was used in 60 acute small craniotomy STA-MCA bypass procedures performed at our hospital from October 2019 to March 2023. Forty five men and fifteen women, average age 72.1 years (45-89 years), refractory to medical therapy, progressive, and symptomatically unstable were included. Average time from cerebral infarction onset to surgery was 9.6 (0-25) days. No postoperative complications such as CSF leakage, infection, or subcutaneous hematoma were observed. Mean operation time was reduced by 41.8 minutes in the five cases immediately after DuraGen® introduction relative to the five cases immediately prior, with all 10 cases performed by the same surgeon. Duraplasty using DuraGen® offers advantages including reduction in complications, time savings due to material flexibility, and no need for sutures, preventing CSF leakage without compressing the STA at the dural penetration site.
