A 48-year-old woman presented with a perineal mass six months prior. She underwent resection under her previous doctor, and it was diagnosed as a malignant granular cell tumor. Histopathologically, the tumor cells exhibited abundant cytoplasm with eosinophilic granules and strongly polymorphic atypical nuclei, proliferating while forming foci. The patient underwent an extended resection with a 2 cm margin ; however, one site tested positive, leading to an additional excision. Approximately one year later, a local recurrence on the left perineum and a metastasis in the left inguinal lymph node were identified. Subsequently, resection of the recurrent lesion and left inguinal lymph node dissection were performed. Postoperatively, a total of 50 Gy radiotherapy was administered to the tumor bed as adjunct therapy. Malignant granular cell tumors are exceedingly rare, and this case is presented alongside a literature review for comprehensive understanding.[Skin Cancer (Japan) 2024 ; 39 : 30-35]
