Two cases of rapidly growing facial squamous cell carcinoma showing early metastasis after radiotherapy despite primary tumor control

Abstract

Cutaneous squamous cell carcinoma (SCC) has a good prognosis if treated early. However, rapidly growing tumors carry a high risk of metastasis. Here, we report two cases of rapidly growing facial SCC in which primary tumor remission was achieved via radiotherapy but metastasis occurred shortly afterward. Case 1 involved a 74-year-old woman with SCC of the right upper lip and case 2 involved an 82-year-old woman with SCC of the left nasal wing, both of which exhibited a tumor growth rate > 4 mm/month. Although the primary lesions disappeared after 55 Gy of radiation, case 1 developed cervical lymph node metastasis after six months and case 2 died of systemic metastasis 11 months after the initial visit. Therefore, SCC with a high tumor growth rate require intensive monitoring and swift intervention because of the heightened risk of early metastasis, although the primary lesion is controlled.[Skin Cancer (Japan) 2026 ; 41 : 7-12]
