Abstract
Schwannomas are benign neoplasms derived from Schwann cells of the neurilemma and arise in the oral and maxillofacial region infrequently. Solitary intraosseous schwannoma of the mandible is rare. Here we report a case of intraosseous schwannoma of the mandible treated by nerve conduit Nerbridge® intraorally. We evaluated sensory nerves using the Semmes-Weinstein monofilaments test (SW-T) and Sensory Nerve Action Potential (SNAP). The patient was a 32-year-old female with mandibular tumor. Orthopantomographic and computed tomographic examination showed a radiolucent, well circumscribed lesion of the right body of the mandible. We performed tumor resection intraorally under general anesthesia, and reconstructed the inferior alveolar nerve with a nerve conduit. Histological examination showed that the tumor cells were arranged in a palisading pattern of Antoni type A cells and type B cells. Some tumor cells showed positive reaction for S-100 protein. Neurosensory disturbance of the mental nerve improved 6 months after surgery using SW-T and SNAP, and approximately normally status was recovered after 12 months. Following neurorrhaphy using Nerbridge®, the postsurgical evaluation revealed marked improvement and significant recovery in sensations of the lower lip.
