Abstract
The gold standard treatment for obstructive sleep apnea （OSA） is nasal continuous positive airway pressure （nCPAP）. However, Japanese patients include those whose facial and jaw morphology contributes to the underlying causes of OSA. In Japan, there are few reports on surgical interventions targeting the base of the tongue. In this report, we present the case of a 44-year-old male diagnosed with obstructive sleep apnea syndrome （OSAS） via polysomnography （PSG）. Based on imaging findings, surgery targeting the base of the tongue was deemed necessary. We performed uvulo-palato-pharyngo-plasty （UPPP） in combination with genioglossus advancement （GA） surgery, which resulted in significant improvement. While UPPP is widely practiced in Japan, its standalone efficacy is not notably high. Therefore, accurate assessment of the site of obstruction, particularly at the base of the tongue, may offer the potential for improvement through surgical intervention. Regarding GA, it may be considered as a viable treatment option and warrants sufficient consideration in treatment planning.
