Abstract

Serous cystic neoplasms of the pancreas (SCN) are relatively rare neoplasms which account for 1% of pancreatic exocrine tumors. SCN is usually followed up because it is mostly benign, but it should be resected when it becomes symptomatic or is suspected of malignancy. We report a case of SCN that was resected because of obstructive jaundice.

A 58 year-old female was referred to our institution for a multicystic lesion of the pancreas head. We diagnosed SCN and followed her up, but she eventually developed obstructive jaundice. We performed subtotal stomach preserving pancreatoduodenectomy after endoscopic biliary tract drainage. The tumor was composed of multiple spongy cysts which contained serous fluid. Histopathological examination showed that each cyst was lined by a single layer of small cuboidal cells and no atypia was proven. The stenosis of the common bile duct was caused not by invasion of malignancy, but by compression of cystic lesions. Finally we diagnosed serous cystic adenoma (macrocystic dominant type).
