Laparoscopic management of common bile duct stones

Abstract
In recent years, laparoscopic common bile duct exploration (LCBDE) has become the procedure of choice in the management of common duct stones. In this article, we discuss the result of LCBDE.
We experienced 419 cases in these 16 years. The result of LCBDE was successful. The success rate of LCBDE was 97%. Mean operating time was 181 minutes and mean hospital stay was 9.7 days. The morbidity rate was 9.1% and mortality rate was 0.2%.
Recurrence occured in 33 patients (7.9%), but recurrence rate was very low in secondary stones compared with previously published reports. This suggests a importance of the preservation of sphincter function. So, we have to avoid unnecessary sphincterotomy. In the multivariate analysis, origin of the stone was significantly related to recurrence.
LCBDE is a feasible approach for CBD stones. It is a skill which should be masterd by the biliary surgeons.
