Abstract
Hepatopancreatoduodenectomy (HPD) has been conducted as one of radical surgical procedures to the biliary lesions such as widely spread bile duct cancer and gall bladder cancer with duodenal or pancreatic infiltration. Even in the era in which several surgical techniques have been developed, the operative results following HPD have been reported with disappointedly high morbidity and mortality. Published article described that the long-term survival outcome of the patients who underwent HPD achieved pathological complete (R0) resection for bile duct cancer is relatively fair. Meanwhile, it was also revealed that the patients with gallbladder cancer after HPD survived shorter time than those with bile duct cancer. To achieve expectedly fair survival following HPD, the preoperative image diagnosis ensuring R0 resection is essential. Hepatobiliary surgeons should collect the data of the patients treated with HPD from several institutes to clarify the most suitable candidates for the procedure as an urgent issue.
