A case of gallbladder torsion associated with gallbladder cancer

Abstract

The case is a 93-year-old woman. She complained of right-sided rib pain while hospitalized for a lumbar compression fracture. An abdominal CT scan revealed an enlarged gallbladder, and she was referred to our department for the purpose of surgical treatment for the diagnosis of acute cholecystitis. A CT examination reexamined at our hospital showed a marked gallbladder swelling, edematous thickening of the wall, and some areas with unclear continuity were recognized. Ascites was observed around the gallbladder, and the gallbladder was twisted counterclockwise about 2 times in the neck. The twist was released, the gallbladder artery and gallbladder duct were cut, then laparoscopic cholecystectomy was performed. Several stones were found inside the gallbladder. In addition, a part of the mucous membrane with widespread necrotic changes was accompanied by polypoid flattened changes, and adenocarcinoma (tub1) was detected by histopathological examination, and gallbladder cancer was diagnosed. There was no problem after the operation, and he was transferred to the previous hospital on the 15th day after the operation. We report a case of gallbladder torsion associated with gallbladder cancer.
