A 66-year-old woman was referred to our department for the examination of a pancreatic cyst. Endoscopic ultrasonography showed a tumor in the bile duct and another duct communicating with the bile duct. Cholangiography revealed the bile ducts forming a circuit, double cystic ducts, and a defect suspected of the bile duct tumor. Communicating accessory bile ducts (CABDs) and a fibrous polyp were diagnosed based on the imaging and pathological findings. After subtotal stomach-preserving pancreaticoduodenectomy, CABDs were histologically confirmed. We report a rare case of CABD with double cystic ducts and a bile duct fibrous polyp.
