The patient was a 64-year-old man with a thickened gallbladder wall on a CT scan during a follow-up for an abdominal aortic aneurysm. Cholestatic adenomyomatosis was suspected, but gallbladder cancer could not be excluded, leading to the decision to perform surgery. Laparoscopic observation initially indicated chronic cholecystitis, but evidence of invasion into the liver bed prompted a diagnosis of gallbladder cancer. Consequently, the procedure was converted to an open laparotomy. The surgical intervention involved hepatic bed resection, extrahepatic bile duct resection, and D2 lymph node dissection. Histopathological examination revealed ring cell-like carcinoma cells proliferating throughout the gallbladder wall, with a few intermediate and poorly differentiated adenocarcinomas. Carcinoma in situ was found in the Rokitansky-Ashoff sinus, confirming a diagnosis of primary gallbladder cancer. Due to numerous lymph node metastases, including in the no. 16 lymph nodes, the patient underwent adjuvant chemotherapy with gemcitabine+cisplatin. One year post-surgery, multiple metastases were detected, and chemotherapy was initiated. However, the patient succumbed to their primary illness 1 year and 6 months post-surgery. Primary ring cell carcinoma of the gallbladder is rare, and resection is infrequently reported. This report presents our experience with a resected case, supplemented by a literature review.
