A male patient in his 70s who had undergone extended left hepatectomy with biliary reconstruction for perihilar cholangiocarcinoma developed a fistula between an intraabdominal abscess and middle hepatic vein (MHV) with anastomotic leakage of hepaticojejunostomy followed by persistent bacteremia. A stent graft was placed on the proximal side of MHV, and coil embolization was performed on the peripheral side of MHV to close the fistula. The abscess subsequently shrank, and he was transiently discharged home although he was readmitted to the hospital due to infection of the stent graft and coil. Therefore, he underwent reoperation to remove them through an incision in MHV. After reoperation, remission of bacteremia was achieved, and he was discharged home. Emergency interventional radiological procedure followed by an open surgery was effective in treating a fistula between an intraabdominal abscess and MHV.
