Interventional radiology (IVR) plays a crucial role in the management of diverse biliary diseases. For post-operative arterial hemorrhage, IVR offers both embolization and stent-graft placement. Embolization is minimally invasive but carries a risk of ischemic complications. Stent-grafts, while technically demanding, preserve arterial flow and minimize ischemia. For portal vein stenosis or occlusion, portal vein stenting effectively restores blood flow and alleviates portal hypertension. Although generally safe, long-term patency and complications remain concerns. Portal vein stent-grafts provide excellent hemostasis for portal vein bleeding. Vascular IVR is integral to biliary disease management, with ongoing technical and clinical advancements expected to further improve patient outcomes.
