A gallbladder mass was incidentally found in an 85-year-old woman with a history of right upper lobectomy and mediastinal lymph node dissection for right lung cancer, on abdominal MRI 10 months after the initial surgery. The lesion was initially diagnosed as gallbladder cancer (T2bN1M0, Stage III B). The patient underwent gallbladder bed resection and regional lymph node dissection. Histopathological examination revealed large cell neuroendocrine carcinoma with features of adenocarcinoma differentiation. The histological pattern closely resembled that of the patient's primary lung cancer, leading to the final diagnosis of metastatic lung cancer to the gallbladder.
Metastasis of lung cancer to the gallbladder is rare, with only 11 reported cases in the literature to date. In this case, differentiating between primary gallbladder cancer and metastasis from lung cancer was difficult. We report this noteworthy case and provide a literature review for further discussion.
