Perioperative management of perihilar cholangiocarcinoma (pCCA) recommends preoperative biliary drainage (PBD), but the choice of drainage method can affect prognosis. This study compared the incidence of recurrent biliary obstruction (RBO) among patients who underwent surgical resection for pCCA and received PBD using nasobiliary drainage (ENBD), conventional plastic stents (cPS), or inside stents (iPS). The results showed that the RBO incidence was significantly lower in the iPS group compared to the cPS group (7.7% vs. 44.1%, p=0.03), and multivariate analysis identified cPS as an independent risk factor for RBO. The findings suggest that iPS may be particularly useful in cases with prolonged preoperative waiting periods or those undergoing preoperative chemotherapy. This study was a single-center retrospective observational study, and further prospective multicenter studies are warranted.
