Abstract
We experienced 2 cases with benign choledochal stenosis caused by severe chronic cholangitis which were difficult to differentiate from malignant stenosis. One case was a 44-year-old male with chief complaint of the jaundice. Following decrease of joundice by PTCD, cholecystectomy and excition of stenotic choledochus were performed. The diagnosis was histologically chronic cholangitis with severe fibrosis. Another case was a 66-year old male of cholelithiasis. After the cholecystectomy, localized stenosis was observed in the middle part of the common bile duct. Cholangioectasis by balloontip catheter in the stenotic site was unsuccessful and therfore, the second operation, choledochojejunostomy (Roux-Y) in which the stenotic site was left was carried out. Biopsy of the stenotic site showed no malignancy and was also diagnosed chronic cholangitis as well as first case. Main lesion in both cases was a chronic cholangitis, however its malignancy was not completely denied. The long term careful follow-up including differentiation from primary sclerosing cholangitis (PSC) will be required in the future.
