Abstract
A 59-year-old man with a history of hyperthyroidism undergoing combined interferon (IFN) alfa2b and ribavirin therapy lost weight and suffered polyuria with elevated serum glucose levels after 4 months of therapy. Examination showed high titer anti-GAD antibody of 27,000 U/ml, but no thyroid abnormalities. HLA typing showed DRB1*0405, DQA1*0303, DQB1*0401 suggesting susceptibility to type 1 diabetes mellitus. IFN therapy was disrupted and he required insulin therapy with continuous high titer anti-GAD antibody a year after type 1 diabetes onset.
