Exploration of a novel pathological factor in peritoneal deterioration induced by long-term peritoneal dialysis

Abstract

Background

In patients with severe kidney failure, the control of electrolytes and filtration of wastes do not function properly. This leads to lethal complications such as heart failure and uremia, thus patients must undergo dialysis.

There are two methods of dialysis, peritoneal dialysis (PD) and hemodialysis.

Although PD can provide a better quality of life, only 3% of patients choose PD because of its inevitable risks, such as peritoneal deterioration.

Various dialysis fluids have been developed to resolve the risks, but these fluids cannot completely suppress them.

Therefore, this study aimed to identify a novel factor promoting peritoneal deterioration, and contribute to the popularization of PD.

Methods and Results

Dialysis fluid containing methylglyoxal (MGO) was administered intraperitoneally to mice three times a week. We found the collagen content in peritoneal walls was upregulated by MGO.

Then, we focused on peritoneal cells as deterioration-promoting cells. We measured the mRNA expression of various factors and found that the expression of factor X was upregulated in a time dependent manner.

Finally, we mixed factor X neutralizing antibody into the dialysis fluid containing MGO and administered them to mice. The collagen content in peritoneal walls was upregulated by MGO, and this was suppressed by antibody.

Conclusion

The amount of factor X in the peritoneal cavity was upregulated in parallel with peritoneal deterioration, and factor X neutralizing antibody inhibited deterioration. These results showed that factor X is one of the main causes of peritoneal deterioration.
