Abstract
 Since the introduction of the electronic medical records, a large amount of data has been stored in each hospital. However, whether those data have been effectively analyzed or evaluated does not reach general agreement.
 For the purpose of the improvement of medical care, we have developed the clinical decision making supporting tool in which the stored data are analyzed and converted to the knowledge base on real time. By this knowledge bases, the candidate of the phrase can be presented on the electronic medical record.
 We conclude that the paradigm shift from the record system to real-time clinical decision supporting tool is important and necessary.
