    Objective:To estimate the effect of replacing sedentary behavior (SB) with low-intensity activity (LPA) on physical performance and activities of daily living (ADL) at the discharge in elderly heart failure (HF) patients with physical frailty using an Isotemporal substitution model.

    Methods:Thirty patients with HF were selected from inpatients whose physical activity was measured using an activity meter with a built-in accelerometer at the beginning of cardiac rehabilitation and analyzed. The effect of replacement of SB with LPA on the Short Physical Performance Battery (SPPB) and the Barthel Index (BI) at discharge was estimated.

    Results:The median SB was 764 minutes and LPA was 75 minutes. The SPPB (B=1.0, p=0.045) and the BI (B=5.2, p=0.039) at discharge was significantly higher when SB was replaced by LPA for 60 minutes.

    Conclusion:Efforts to replace SB with LPA in patients with HF may improve physical performance and ADL at discharge.

    Aim:This study aimed to determine the effects of malnutrition on the development of intensive care unit-acquired delirium (ICU-AD) in older patients with heart failure and physical frailty.

    Methods:From September 2020 to August 2021,85 patients with heart failure, aged 65 years or older were enrolled (84.2±7.9 years, 42% males). The patients were divided into four groups for comparison:normal, low nutrition, frailty, and comorbidity.

    Results:ICU-AD occurred in 37% of the patients and was significantly higher in the comorbidity group than in the normal group (57% vs. 9%, p<0.01). Multivariate logistic analysis showed that the combination of malnutrition with frailty was significantly associated with ICU-AD (odds ratio=3.18, 95% confidence interval 1.05-9.56, p=0.040).

    Conclusion:Malnutrition in older patients with heart failure and physical frailty may increase the risk of developing ICU-AD.

    Aim:We aimed to clarify the association between the preoperative 6-min walk distance (6MWD) and rehospitalization within one year in patients undergoing transcatheter aortic valve implan-tation (TAVI).

    Methods:The analysis target was 356 patients who underwent elective transfemoral approach TAVI (TF-TAVI) between May 2018 and December 2020. The cut-off value predicting readmis-sion was calculated in order to compare the readmission rate. Additionally, three models were created by a multivariate analysis in order to examine the association between readmission and preoperative 6MWD.

    Results:The preoperative 6MWD cut-off value predicting readmission was set for 168 m and the re-admission rate increased significantly in the group of less than 168 m. Even after adjusting for each factor, preoperative 6MWD was still extracted as a factor predicting readmission.

    Conclusion:Preoperative 6MWD is associated with readmission within 1 year after TAVI.
