One hundred and fourteen trochanteric fractures of the femur were analysed. The ratio between males and females was 3.07. The average age of the patients was 78.6 years for both males and females. Low energy trauma (“simple fall” due to slipping, tripping, stumbling, or due to dizziness.) was the cause in 63 hips (73.2%) in female, in 16 hips (57.1%) in male. Seventy two percent of the females and 62% of the males were injured indoors. Sixteen fractures occurred while the patients were hospitalized for other reasons. There were 86 patients (76.3%) with preexisting diseases which were possibly related to the fracture. The cost of the treatment was \1, 418, 000. We suspected that the cost of the treatment of the trochanteric fractures in Japan was \4, 679, 400, 000.

For adult tibial condylar fracture regarded as an intractable intra-articular fracture, the very important plan of treatment consists in acquiring articular supportability and motility. We investigated the results of treatment for cases of tibial condylar fractures that had undergone operative treatments at the Department of Orthopedic Surgery, Kagawa Central Hospital during the past 9 years. The related factors that seemed to have influenced the result were also studied.
Twenty-eight patients (28 knees) aged 16-75 were followed up for not less than a year (4 years and 4 months on the average). Clinical results were evaluated based on the criteria of Hohl & Luck and the “SANDAIGAKU” Tentative Plan.
From the stand-point of operative procedures, the following should be taken into consideration: 1) accurate reduction of the tibial condylar articular surface, 2) recovery of the alignment of the anatomical lower extremity and 3) accurate diagnosis and repair of associated soft issue injury.
The following are regarded as the cases for poor results: 1) severe types of the fracture, 2) technical problems, 3) past history of osteoarthritis of the knee, 4) advanced age and 5) multiple traumas.
The combination of these factors seemed to dictate the result of the trertment for the fracture.

All five carpometacarpal dislocations including thumb are extremely rare. Following is a case report of such an injury.
A 41 year old woman was involved in a motorcycle accident, sustaining injuries to the right hand. Upon her hospital admission, there were lacerations on the dorsum of her thumb and thenar eminence. Radiographs showed dorsal dislocations of all of the five carpometacarpal joints and a fracture of the trapezium. Closed reduction was unsuccessful, and therefore, an open reduction was carried out, stability was obtained by fixation with Kirschner wires. Eight weeks later, a dynamic splint was used after the wires were removed. Four years after injury, fingers and wrist motions were equal bilaterally and grip strength was normal.
One of the reasons why this injury is extremely rare is the difference between the mechanism of the carpometacarpal dislocation of the thumb and that of the ulnar four fingers.
