Current issues and prospects of critical-paths in Japan were reviewed. Critical-paths arepredominately applied in surgical operation and diagnostic procedure in surgery, orthopedicsurgery, gynecology and urology. Critical-paths combined with clinical practice guidelines and evidenced-based medicine are recommended. Critical path is also discussed under the concept of outcome management in health care. Outcome management in health care is a concept of management by health outcome such as clinical outcome, financial outcome, hospital length of stay and patient satisfaction. It is emphasized to reexa mine critical path from the viewpointof outcome management.

Impact of Critical-path of educational program for Diabetic Inpatient was examined from thefollowing points of view, collaboration approach of co-medical staff team, standarizaion of nursing process and hospital management.
Comparison between critical-path applied group (nineteen patients) and non applied group (twenty-eight patients) was done on labo-data, hospital fee per day and averaged length of stay. As results, statistical difference was not detected on hospital fee between two groups. Differences was however, detected on average length of stay and Comprehensive test. Critical path applied groupe showed 14.6 days in average length of stay and comparing to 25.2 days of control goup.
