    In order to improve safety management of multiple medications in patients with cardiovascular diseases (CVDs), we conducted the incident report analysis and the inpatient questionnaire survey. Prevalence of incident report related to taking medication was approximately 44%. Among 100 inpatients with CVDs (mean age 71.4), all judged as able to manage their medications by themselves, the questionnaire regarding ingestion method, tablet dropping rate, and awareness of finger disabilities were performed. Sixty-seven percent of patients counted tablets and 85% took all tablets together. Fifty-three percent reported accidentally dropping tablets, occurred more frequently over age 65, regardless of finger disabilities. Fewer cases dropped tablet when taking directly from one-dose packages or containers than taking by hand. Medication guidance based on patients’ age and finger abilities would reduce adverse incidents with polypharmacy and improve safety management.
