Investigation of Sisomicin, a new aminoglycoside antibiotic, performed in 42 research facilities in Japan led to the following results.
1) The antibacterial spectrum of Sisomicin was almost similar to that of Gentamicin (GM).
2) In sensitvity tests of clinical isolates, Sisomicin exhibited slightly more active against Pseudomonas aeruginosa and Klebsiella, and conversely less active against Serratia in comparison with GM.
3) The therapeutic activity of Sisomicin in mice infected with Pseudomonas aeruginosa was superior to that of Tobramycin (TOB) when challenge dose was high, in spite of its lower antibacterial activity against Pseudomonas aeruginosa.
4) Pharmacokinetics of Sisomicin were almost similar to those of GM with respect to the concentrations in the serum, urine, and tissues.
5) Nephrotoxicity of Sisomicin in rabbits was estimated to be similar to that of GM. In monkeys, nephrotoxicity of Sisomicin was less than that of GM.
6) Ototoxicity of Sisomicin in guinea pigs was found to be less than that of GM, while the vestibular disturbance of the drug was more severe than that of GM.
7) Clinical evaluations of Sisomicin were investigated on 160 cases in the fields of internal medicine and its related area, and on 284 cases in the fields of surgery and its related area. The efficacy rate was 70.1% and 100% respectively in the fields of internal medicine and pediatrics. And the efficacy rate of 65.1% was obtained in the fields of surgery, orthopedics and neuro surgery. The efficacy rate was 66.9%, 90.9%, 86.7% and 80.0% respectively in the fields of urology, obstetrics and gynecology, otorhinolaryngology and ophthalmology.
The efficacy rate was 64.9% and 71.6% respectively on 77 cases of respiratory tract infections and 232 cases of urinary tract infections.
8) Side effects and abnormalities in laboratory findings with the drug were observed in 43 cases out of 453 cases (9.5%).
No severe side effects have been reported up to now.
9) In conclusion, Sisomicin is one of favorable drugs among aminoglycoside antibiotics.
