A 68-year-old man was referred to our clinic for management of abdominal pain and diarrhea during hospitalization for open fractures of the right lower leg. Computed tomographic imaging showed distal ileum wall thickening, and single-balloon endoscopy showed a circumferential segmental ulcer; moreover, the patient presented with hypoalbuminemia. Intravenous technetium-99m human serum albumin diethylenetriamine pentaacetic acid scintigraphy revealed protein leakage at the site of the ilium lesion. Thus, the diagnosis of protein-losing gastroenteropathy caused by ischemic enteritis was made. The patient did not respond to conservative treatment; therefore, ileocolic resection was performed, following which his clinical symptoms and hypoalbuminemia resolved. Clinicians should be aware that ischemic enteritis may lead to protein-losing gastroenteropathy.
