Although endoscopic resection for ulcerative colitis (UC)-associated neoplasia (UCAN) has recently been performed, consensus has not been achieved regarding its indication and treatment method. The indication for endoscopic submucosal dissection (ESD) of UCAN in our institution is as follows: a lesion diagnosed as low-grade dysplasia via preoperative biopsy with an endoscopically visible demarcation, several biopsy specimens from the surrounding mucosa that were negative for dysplasia, and the patient has either UC in endoscopic remission UC. ESD for UCAN is technically difficult because of the presence of severe submucosal fibrosis; therefore, reliable strategies and effective devices are necessary.
