A 61-year-old man presented to another hospital in May 2020 for evaluation of itching and brown-colored urine. Blood tests and contrast-enhanced CT revealed obstructive jaundice secondary to a duodenal papilla tumor, and he underwent endoscopic retrograde cholangiopancreatography. Histopathological evaluation of a biopsy specimen and cytological analysis did not show evidence of malignancy, and the patient was referred to our hospital for further management. The patient was diagnosed with adult T-cell leukemia/lymphoma (ATLL) based on endoscopic ultrasonography guided fine-needle biopsy (EUS-FNB) of the papillary tumor. We report a rare case of ampullary ATLL diagnosed by EUS-FNB, which enabled appropriate treatment.
