Gastric adenocarcinoma and proximal polyposis of the stomach (GAPPS) is a genetic disease that causes gastric cancer due to fundic gland polyposis. Pathogenic variants in promoter 1B of the APC gene have been reported. However, since GAPPS is a rare disease, the number of reports remains very small, recognition is poor, and insufficient studies detail its natural history and long-term prognosis. This article explains the disease concept and epidemiology of GAPPS, focuses on its endoscopic imaging findings and histopathological characteristics, and discusses the current status and future challenges in daily clinical practice.
