Abstract

A case reported here is 39 years old male with gastric carcinoid showing multiple polyps on the gastric body. The patient had no symptom and was checked by X-ray examination at the mass screening for gastric cancer. Endoscopic examination following mass screening revealed multiple polyps on the body and carcinoid histologically. Labora-tory data of the patient shoeds achlorhydria and hypergastrinemia. Any metastatic lesion was not observed by X-ray and/or CT examination. In the resected stomach, 6 polypoid lesions were observed. Many endocrine cell micronests (ECM) and carcinoid cell mass were diffusely observed in the deep submucosal layer of the gastric body. Resected gastric mucosa revealed severe glandular atrophy and moderate metaplasia in the fundic glandular area. Diffuse G cell hyperplasia was also observed in the pyloric glandular area. Prolifirated carcinoid cell mass showed positive reaction for serotonin and other 6 hormones immunohistochemically. Electron microscopy showed endocrine granules with round, oval and pleomorphic shapes. According to the theory in that hypergastrinemia may lead to hyperplasia or carcinoid formation of ECL cells, hypergastrinemia will be considered to play some important role to make carcinoid formation.
