Abstract

The Hospitalist Team within the Division for Youth Support of the Certified Family Physician Group of the Japan Primary Care Association, organized workshops at the Autumn Continuing Medical Education Seminar and the Annual Conference focusing on the theme of outpatient collaboration between hospitals and clinics, a topic that has received little attention to date.

The discussions identified several challenges, including differences in prioritization of information, psychological barriers, lack of clarity in role delineation, and insufficient structural support. Potential solutions to these challenges were explored at both the individual and community levels, underscoring the importance of sustained dialogue.
