This paper describes the analysis of a fire accident caused by the leakage of a large amount of oxygen gas in a university. Observation of the fracture surface of a ruptured oxygen tube with a scanning electron microscope provided undisputable evidence that ductile fracture of the tube occurred. Fire-cause investigation revealed that the fire accident was probably owing to the lack of safety-management system in the university. In order to improve the safety management in the university, accident scenarios which are useful for analyzing accidents were made using accident causes, human actions, and their results.
