A Case of Epithelialization Achieved with Application of Perifascial Areolar Tissue (PAT) Grafting and EPIFIX for the Treatment of Diabetic Foot Ulcer with an Exposed Tendon

The treatment of diabetic foot ulcers involving exposed tendons and bones poses significant challenges owing to insufficient blood supply to these surfaces, which limits the applicability of dehydrated human amnion/chorion membrane allografts (EPIFIX). The successful epithelialization of a dorsal diabetic foot ulcer exposing the extensor tendon by combining perifascial areolar tissue grafting with EPIFIX application in a 61-year-old man is described. Perifascial areolar tissue grafting was performed on the tendon, and two weeks later, EPIFIX was applied weekly for 12 weeks as the granulation tissue developed. Complete epithelialization of the wound was observed. The patient subsequently expressed satisfaction with both the functional and cosmetic outcomes.
