Follow-up study was performed in 56 cases with Benign Monoclonal Gammopathy (BMG). The patients were followed up for one to six years. There were 17 secondary and 33 primary BMG. 22 out of 56 cases had gastrointestinal disease, especially hepatobiliary diseases. Total protein level ranged from 6.5 to 9.0g/dl. No value exceeded 9.0g/dl. In 8 of 50 cases ESR was over 500mm/hr. M protein in 18 cases exceed 300mg/dl. Prognosis was excellent except in 7 cases, who died of cancer, congestive heart failure, or cerebral infaction. Four cases are discussed as interesting cases : 1) Transient BMG associated with liver cirrhosis. 2) Uterine cancer, with BMG appearance shortly before death. 3) Cholelithiasis with persistent BMG after cholecystectomy. 4) Hepatic cirrhosis with hepatoma two years later. These results that BMG might be an early sign of progressive disease. However the development of BMG into multiple myeloma is exceptional.
