Abstract
A 19-years-old man, one member of the twins was admitted to Iwakuni National Hospital for butterfly rashes, palmar and plantar erythema in April 2, 1968, We suspected systemic lupus erythematodes (SLE), but LE cells were negative. Paramethasone resulted in disappearance of skin rashes. At the time of his second admission we confirmed SLE for reappearance of butterfly rashes with positive LE cells and 26 titers of anti-nuclear antibody. The renal, liver and skin specimens showed no lupus lesions by light-micro-scope. A maintenance dose (10 mg predonisolone) had been continued, but in July 1, 1970 photosensitivity aroused fever and polyarthralgia. In August 2, 1971. anti-nuclear antibody showed 27 titers but there was no remarkable sign or symptoms. He suffered from fever, soar-throat and albuminuria, and then developed slurred speech and admitted in December 1, 1971. Marked albuminuria, psychosis and pneumonitis progressed and he died of uremia in May 2, 1972.
Familiar diathesis of collagen disease was examined. His parents and sister had no illness, but high gammaglobulinemia was found by the serological examinations of the another member of the twins.
