Emergency care system in Japan commonly consists of the emergency care, initial care, intensive care, and pre-hospital care. The system depends on the characteristic of department, hospital roles, and responsivity. Our hospital have provided pre-hospital care, emergency care, trauma and emergency general surgery, critical care, and surgical rescue for critically ill patients seamlessly. There are some benefit of double boards certificated physician (emergency physician as well as surgeons) as follows : (1)prompt decision making to treat patients with high urgency condition in ER, (2)seamless care from prehospital care to intensive care, (3)best practice for severe patients in surgical intensive care unit, (4)appropriate perioperative management of patients with severe co-morbidity, (5)surgical rapid response in hospital. Acute care surgeons are a social necessary. In addition, residents who want to obtain double boards (emergency medicine and surgery) are increasing. It is important for us to spread the concept of Acute Care Surgery including emergency medicine in Japan.
